Reporting Concerns
To maintain an inclusive and safe learning environment, the university has tools in place to respond to incidents that impact our students, employees, or guests of the university.
Below are links for reporting general concerns and specific incidents.
What happens to reports?
When a report is submitted, it is reviewed by designated personnel who evaluate and respond, as appropriate, to each incident. Each response will be in accordance with 91ÖÆƬ Polytechnic, Universities of Wisconsin, and state policies or statutes.
Submitting an incident form is not the same as filing a police report or pressing charges. To submit a police report, contact University Police.
Submitting an incident report does not always result in disciplinary action by the university.

The Behavior Intervention Team (BIT) works to keep 91ÖÆƬ safe and healthy by identifying, assessing, and intervening early with students whose behavior raises serious health or safety concerns for themselves or others in the campus community.
If you've observed behavior that concerns you for someone's well-being, is disruptive or threatening, or could lead to harm to self or others, you can report that . This report is sent to the Dean of Students Office for review.
91ÖÆƬ is committed to preventing all forms of sexual misconduct, including sexual assault, dating and domestic violence, stalking, and sexual harassment. These are university policy violations when they involve students, faculty, or staff — on campus or off.
If you believe you've witnessed or experienced sexual misconduct, no matter how minor it may seem, you can report that . This report is sent to the Title IX Coordinator for review.
Hazing is any intentional, knowing, or reckless act against an individual — regardless of their willingness to participate — that endangers their physical, mental, emotional, or psychological health for the purpose of initiation into, affiliation with, or continued membership in an organization or group. Bias incidents are acts of bigotry or hate targeting someone's actual or perceived race, ethnicity, nationality, gender, sexual orientation, gender identity/expression, religion, or disability. Hate crimes are bias-motivated acts — such as violence or vandalism — that also violate criminal law. Learn more about the Education and Resource Response Team here.
If you believe you've witnessed or experienced hazing, a bias incident, or a hate crime, you can submit a report . This report is routed to the Dean of Students Office for review and further action. Learn more about the complaint process here.
If you believe a student has been missing for more than 24 hours, submit a Missing Student Incident Report. You can also notify your RA or RLC, and should call 911 in an emergency.
Anyone can at any time they believe a student has been missing for an unusual length of time. If you believe a student is a threat to themselves or others, contact University Police immediately.
Wisconsin requires all UW System employees to report incidents of child abuse or neglect to appropriate officials or local law enforcement.
If you observe, or learn of, an incident or threat of child abuse or neglect — no matter how small it may seem — you can . This report goes to University Police.
Use this report type for incidents at a university youth program that don't involve sexual misconduct or child abuse or neglect. University youth programs include camps, clinics, classes, events, or other activities involving minors that are hosted, sponsored, or operated by the university.
Report incidents involving youth or minor participants here, including safety concerns, supervision issues, policy violations, or other inappropriate or concerning behavior.
Reports submitted are reviewed by the university officials responsible for youth program oversight and participant safety.
If your incident doesn't fit any of the categories above, file a . Include as many details as you're comfortable sharing. The Dean of Students Office will receive the report and route it appropriately upon review.
